Chronic Disease Management at Jindabyne Medical Practice
Patients with chronic conditions such as diabetes, asthma, osteoarthritis, heart disease and chronic back pain are eleigible for GP Management Plans
These plans are developed together with the patient and identify chronic health problems and the actions to address these problems. For example: with osteoarthritis - lose weight, be active, medication, seek help of physiotherapy, dietician, exercise physiologist and eventually involve an orthopaedic surgeon if joint replacement is considered.
When more than 2 health providers are involved in the care of a patient with a chronic condition, we can turn the management plan into a team care arrangement. You will then have access to 5 Medicare subsidized allied health services (physiotherapy, podiatry, chiropractor etc).
It is also a great opportunity to sit down with your GP to make sure all your health records are up to date that you hav been offered the health checks you may need ( eg mammograms, bowel cancer screening).
If you would like an GP Management Plan please call reception so we can book you in. Thet usually take 30 minutes ( ask for a long appointment) and are bulk billed. Some of our regular patinets may be on a recall list and will get reminders when the plans and reviews are due.

Our GPs can also prepare Mental Health Care Plans which are specifically for those needing mental health support. Eligible patiets can access up to 10 rebateable sessions with a psychologist under this plan.
